No, they don't. Its a way to keep your herbalist and doctor busy. But I have had spells where mine didn't get ill as often. Yesterday I never even used my free doctors visit. I was able to use the potions I had made.

I recommend explorer for the 2nd. That way you can gather stuff and sell to make miney to open the next 2 slots. But, there is no wrong way. Just some jobs require stuff you can't get yet. And they can have more than 1 job, which you can switch every 2 hours now.
