you can get them from spinning the cursed wheel during the harvest carnival halloween event, but there are also a few items that, if used on a specific villager, will give them vampirism, lycanthropy, zombipathy, and glitterosclerosis (a newer ailment)
some items that can give you these illnesses are:
vampirism: vampiric blessing amulet, faeli's cursed amulet, dark bardiche and the dark scythe
lycanthropy: werebeast gauntlet
glitterosclerosis: sludge potion
zombipathy: n/a
also note that these are temporary illnesses
